It’s important to review your legal notices to stay informed about your rights, responsibilities, and any changes to your benefits or coverage. Legal notices provide critical information about compliance with federal regulations, plan updates, privacy practices, and how your benefits are administered.

Medical Plan

This benefit is offered by your employer, not Mark III. For questions, please contact your employer directly.

Health insurance helps provide coverage for preventative care, treatment, pharmacy, and other medical services.  Health insurance protects you from paying the full cost of medical services when you’re injured, sick, or have other medical costs.  Also, in most cases, health insurance is required by law in order to avoid a fee when filing your federal tax return.

*Effective July 1, 2022, Transparency in Coverage regulations require health plans to publish their in-network negotiated rates and out-of-network allowed amounts in the form of machine-readable files (MRF) made available on a public website. To access, please go to https://www.anthem.com/machine-readable-file/search.

Health Savings Account (HSA)

A health savings account (HSA) is a tax-advantaged savings account that can be used to pay for medical, dental, vision and other qualified medical expenses now or later in life. Contributions are limited annually and the funds can even be invested, making it a great addition to your retirement portfolio.

Flexible Spending Accounts

A Flexible Spending Account is a special account you put money into that you use to pay for certain out-of-pocket health care costs. You don’t pay taxes on this money. This means you’ll save an amount equal to the taxes you would have paid on the money you set aside.

Dental Plan

This benefit is offered by your employer, not Mark III. For questions, please contact your employer directly.

Proper dental care is important and taking care of your oral health is an investment in your overall well being. By seeing your dentist regularly, you can catch dental problems before they get too serious and require more extensive and expensive procedures.

Vision Plan

This benefit is offered by your employer, not Mark III. For questions, please contact your employer directly.

A routine eye exam is important not only for correcting vision problems, but for maintaining healthy eyes and overall health wellness. Eye care providers are trained to teat for and diagnose a variety of health issues, not just eye problems. Take the time to get to know your vision plan and start experiencing healthy eyes and healthy living.

Cancer Plan

With the rates of cancer increasing, cancer treatment can lead to unexpected expenses to add to your financial burden. Cancer coverage fills in the gaps your medical insurance doesn’t cover. Benefits are paid directly to you and can be used for a variety of purposes.

Accident Plan

Accident insurance helps you pay for the medical and out-of-pocket costs that you may incur after an accidental injury. This includes emergency treatment, hospital stays, and medical exams, and other expenses you may face, such as transportation and lodging needs.

Hospital Indemnity Plan

Hospital indemnity insurance is a supplemental policy designed to provide additional financial support during hospital stays. It pays a fixed cash benefit directly to the insured for each day they are hospitalized, helping to cover out-of-pocket expenses that traditional health insurance may not fully address, such as deductibles, copayments, transportation, and lodging costs for family members.

Critical Illness Plan

Critical illness insurance is a type of coverage that provides a lump-sum cash benefit upon the diagnosis of a serious illness specified in the policy, such as cancer, heart attack, or stroke. This benefit can be used at the policyholder’s discretion, helping to cover medical expenses, treatment costs, and other financial burdens like mortgage payments, utility bills, and everyday living expenses.

Short & Long-Term Disability Plan

Disability insurance is a crucial form of coverage that provides financial protection by replacing a portion of an individual’s income if they become unable to work due to illness or injury. It helps ensure that essential expenses such as mortgage payments, utility bills, and daily living costs can still be met during a period of disability.

Whole Life Plan

Whole Life Insurance is an ideal complement to any group term or optional term life insurance your employer might provide. It will help protect your family today, and more importantly tomorrow.

VRS Optional Term Life Plan

This benefit is offered by your employer, not Mark III. For questions, please contact your employer directly.

Needs change over time and flexible term insurance can mirror those changes. It acts as temporary insurance to meet your temporary needs. The necessity to provide protection for a family starts out high, but generally diminishes over time as children reach maturity and savings plans accumulate.

Hybrid Disability Plan

This benefit is offered by your employer, not Mark III. For questions, please contact your employer directly.

VACORP Hybrid Disability Plan provides some income protection if you can’t work because of physical disease, mental disorder, injury or pregnancy. You are enrolled in this program because your Employer opted out of the state disability program.
